My CMEs’ page is designed to provide access to an individual’s participation in Suburban Health Organization’s (SHO) CME activities. Individuals registering for a SHO CME activity will be asked to set up a personal CME account.  Once a personal CME account has been set up, individuals may register for activities, monitor their participation, and access their credits online. Individuals who have participated in a SHO CME activity already have a personal CME account.

My Profile Set Up for New Registrants

Individuals who have not participated in a SHO CME activity will need to set up an account through My Profile to register for a SHO CME activity.  To set up an account, click on My Profile on the left side of the screen.  A valid email address and password must be provided to set up an account. Please use the same email address and password for each registration to avoid multiple accounts.

My Profile for SHO CME Participants

Individuals who have participated in a SHO CME activity already have a personal CME account.  To access a profile, go to the My Profile link on the left side of the screen. A valid email address and password is required to access an account. Please confirm or change information as appropriate.  If accessing the wrong account, please notify Terri Neaderhiser by telephone at 317-692-5222, ext 283, or by email at cme@suburbanhealth.com.

My Registrations

Individuals may access a list of registrations by going to the My Registrations link located on the left side of the screen. A valid email address, password, and a date range for the activity registrations must be provided to obtain the registration list. Please use the same email address and password for each registration to avoid multiple accounts.

My Transcripts

Individuals may access a transcript of activities participated in by going to My Transcripts link located on the left side of the screen. A valid email address, password, and a date range for the activity dates must be provided to obtain a transcript.

Request a Certificate

Individuals may request a certificate for specific activities by completing and submitting the form available by going to the Request Certificate on the left side of the screen. Certificates will be emailed to the participant within 30 days of the request.
